CF Industries partners with Blue Cross Blue Shield of Illinois (BCBSIL) to provide you the option between two comprehensive PPO medical options — the Advantage PPO with HSA and the Standard PPO.
Both plan options include:
- In-network preventive care covered at 100%, no cost to you
- The same network of providers and facilities
- Comprehensive coverage for the same services; you pay the full cost of services until you meet your deductible, which varies based on the option you choose
- Paying 10% of covered services (coinsurance) once you meet your deductible
- Prescription drug coverage provided through Prime Therapeutics
Advantage PPO with HSA
- The Advantage PPO with HSA has a higher deductible and out-of-pocket maximum.
- The monthly premium is lower in this plan.
- This plan can be paired with a Health Savings Account (HSA).
- CF Industries also contributes to your HSA to help cover eligible medical expenses.
Please note: You can still participate in a Limited Purpose HCFSA even if you are enrolled in the Advantage PPO with HSA medical option. For more details on a Limited Purpose HCFSA, visit the Flexible Spending Account page.
Standard PPO
- The Standard PPO has a lower deductible and out-of-pocket maximum.
- The monthly premium is higher in this plan.
- This plan cannot be paired with an HSA but is eligible for a Flexible Spending Account (FSA).

* If you have family coverage, any covered family member or combination of family members must satisfy the family deductible before the plan pays benefits, even if only one person uses the plan.
** When one family member satisfies the individual deductible, the plan pays benefits for that individual. Then any covered family member or any combination of family members can satisfy the family deductible.
